We will send you a questionnaire that will help us decide if this service is right for your needs.

Insurance companies have different policies and it is important to understand your insurance coverage. 

​

If you have medical insurance and need dental implants or other major dental work (see the list on the ABOUT page) you may qualify for medical insurance benefits.

​

*Medicare and Medicaid do not pay for these services at this time.

​

If you have medical and dental insurance, medical is considered primary, dental is secondary.

​

You need to have an itemized treatment plan from your dental care provider (it's okay if they only provide  you dental codes) in order to submit to the insurance company for a pre-treatment estimation of how much they will pay.

If you have paid out-of-pocket for medically necessary dental treatment (see list in the ABOUT page) in the past 12 months, you could qualify for a reimbursement from your medical insurance company. You must have an itemized statement from your provider.  

 

*Many insurance companies have a 12 month filing limit. Some are less. You should be able to find out this information from your insurance company.
